BILATERAL STRAIGHT LEG RAISE TEST

TEST POSITIONING

 

The client lies supine with both hips and knees extended.

 

The therapist is standing with the distal hand or forearm around or under the client's heels and the proximal hand on the client's distal thighs (anteriorly) to maintain knee extension.

ACTION

 

With the client completely relaxed, slowly raise the legs until pain or tightness is noted.

POSITIVE FINDING

 

Low back pain occurring at hip flexion angles less than 70 degrees is indicative of sacroiliac joint involvement.

 

Low back pain occurring at hip flexion angles greater than 70 degrees is indicative of lumbar spine involvement.

 

CONSIDERATIONS & COMMENTS

 

The therapist must utilize proper body mechanics when performing this test in order to avoid injury secondary to lifting the weight of both legs.

 

The therapist should also note any excessive pelvic motion that may indicate the client's discomfort and/or mechanical compensation associated with the test.
